Hypertension and Multiple Medications Pilot
 
Description
 
This project addresses an innovative shift in the practice of community pharmacists to help meet patient care needs and decrease total medical costs. Studies have documented successful outcomes associated with pharmacist intervention as part of the disease management of chronic conditions such as asthma and diabetes. 
 
Purpose
 
The purpose of this pilot is to improve patient outcomes and address the burden of rising health care costs in Texas. This study aims to determine the clinical and economic value of a community-based MTM program among Medicaid patients suffering from chronic hypertension taking several concurrent medications. The program is a wellness program designed to help build collaboration between physicians, pharmacists and patients with hypertension in order to address a spectrum of disease states including cardiovascular disease, diabetes, and metabolic syndrome.
 
Objectives include assessing the resolution of medication related problems and monitoring clinical markers including blood pressure, fasting blood glucose, lipid status, and the number of emergency room visits.
